Under the Income Tax Act, you have to give your social security number (SIN) on request to anyone who has to prepare an information slip for you. If you do not have an SIN, you can apply for one through any Human Resources Centre of Canada.

Instructions for completing your tax return

You do not have to attach this information slip to your income tax return. File it with the working copy of your return in case we select your return for review at a later date.

This information slip reports the amount paid or credited to you for securities you disposed of or redeemed during the year indicated. These transactions may be on account of income or capital. For more information, see Interpretation Bulletin IT-479R, Transactions in Securities. You can get a copy of this bulletin from any tax services office or tax centre.

Report transactions on account of capital on Schedule 3. You can report transactions on account of income as either business income (line 135 of your income tax return) or, for debt obligations in bearer form, as investment income (line 121 of your income tax return).

You may have to pay your taxes by installments. For more information, contact your tax services office.
